A few months ago, Roni & I attended a SportsVision diploma, after being asked by many of you about glasses for shooting, playing golf and fishing.We learnt about how to train your eyes for sport and how we can improve your game with certain spectacle lenses and prescriptions. Retinal nerve fiber layer atrophy is associated with physical and cognitive disability in multiple sclerosis

Abstract

Background

Studying axonal loss in the retina is a promising biomarker for multiple sclerosis (MS). Our aim was to compare optical coherence tomography (OCT) and Heidelberg retinal tomography (HRT) techniques to measure the thickness of the retinal nerve fiber layer (RNFL) in patients with MS, and to explore the relationship between changes in the RNFL thickness with physical and cognitive disability. We studied 52 patients with MS and 18 proportionally matched controls by performing neurological examination, neuropsychological evaluation using the Brief Repetitive Battery-Neuropsychology and RNFL thickness measurement using OCT and HRT.

Results

We found that both OCT and HRT could define a reduction in the thickness of the RNFL in patients with MS compared with controls, although both measurements were weakly correlated, suggesting that they might measure different aspects of the tissue changes in MS. The degree of RNFL atrophy was correlated with cognitive disability, mainly with the symbol digit modality test (r = 0.754, P < 0.001). Moreover, temporal quadrant RNFL atrophy measured with OCT was associated with physical disability.

Conclusion

In summary, both OCT and HRT are able to detect thinning of the RNFL, but OCT seems to be the most sensitive technique to identify changes associated with MS evolution.

Sadly, on the night of Saturday 4th, one of our shop windows was smashed by a brick. Luckily, only one sheet of glass was smashed so they were unable to get into the shop or take anything.

Headaches, Wakefield

I have just attended a lecture about headaches given by Stephen Butterworth, Consultant Neurologist at Pinderfields Hospital. He discussed the most common causes of headaches, including migraines, medicines over use headaches, tension headaches and more. It was very interesting, as I regularly have patients who come and talk to me about their headaches, as they can be associated with eye strain. Around 50% of them are eye strain but 50% are not, so it was good to discuss the symptoms of non-ocular headaches and the reasons for why they occur. This helps us when we have a patient with headaches but we cannot find eye strain – we can then refer appropriately to the right professional.

Everything's Lovely and healthy!

Here at Pollards Optical Dental Clinic in Wakefield, a half an hour intial assessment with Dr Hassan will allow him to inspect the following areas in the mouth:

  1. We check the jaw joint to make sure that it is healthy, stable, and pain free.
  2. Check to see if the muscles of the jaw and face are sore and causing headaches.
  3. We check all the soft tissues for the presence of Oral Cancer.
  4. We check the gums for the presence of Gum disease, i.e bleeding gums, loose teeth etc
  5. We look for signs of tooth wear caused by clenching and grinding and acidic foods.
  6. We look for fillings that might be leaking or have decay around them.
  7. Include all necassary small radiographs or X-rays to help diagnose any issues you may have.
  8. We check too see that your bite patterns are stable and not contributing to pain in the jaw joint.
  9. Inspect the gums for signs of recession.
  10. Look with high power magnification for the early development of cracks with the teeth.

The above are just a few of the main things that Dr Hassan looks at during our initial inspection appointment. At the end of the appointment we will give you a written estimate of what treatment you require and how much the treatment will cost.
